Gospel Community Church requires waivers for various events. if instructed, Please fill out the appropriate forms below.
If you are under 18, please have your parent or guardian fill out the Minor Medical Release Form. Please note this form is valid January 1, 2025-December 31, 2025.
If you are under 18, please have your parent or guarding fill out the Minor Photo Release Form. This form does not expire.
If you are 18 or older, please fill out the Adult Medical Release Form. Please note this form is valid January 1, 2025-December 31, 2025.
